Defect in immunity leading to tuberculosis infection among children'

Mangaluru, Jan 16: Department of pediatric hematology and oncology, led by Dr. Harsha Prasada L, KMC Hospital in association with National institute of Immunohematology , Mumbai and Imagine Institute, Human genetics of infectious disease team at Paris, France have diagnosed that tuberculosis can also be caused among children due to gene defect. Recently KMC Hospital witnessed two rare cases of 2 year and 6 year old children suffering from tuberculosis due to genetic defect in immunity.

Two year old Shreya (name changed) was brought to the Hospital in a critical condition. She had multiple lymph gland swelling with discharging sinuses and fever. She has had developed lymphadenitis due BCG vaccination and was treated with ant tubercular medicines for 6 months during her first year of life. Few months later the lymph node reappeared with high fever.

Lymph node biopsy showed multiple acid fast bacilli suggestive of tubercular infection. She was diagnosed with Mendelian Susceptibility to Mycobacterial Disease (MSMD that is an inherited condition due to defect in T lymphocytes, a type of white blood cell. Patients with MSMD have increased susceptibility to systemic infections with weakly virulent non-tuberculosis mycobacteria including the Bacillus Calmette-Guérin (BCG) vaccine strain. These conditions are common in children born to parents who married within the family.

Another 6 year old girl was presented with tubercular infection in brain and in the abdomen along with oral thrush and fungal infection of the nails. She was also found out to have low number of T-lymphocytes which is responsible for body defense against infection. Further tests revealed that she had a rare condition called ROR gamma T mutation. In this condition there is a defect in the body immunity against tubercular infection and Candida.

Speaking on the occasion Dr. Harsha Prasada L, Consultant, Pediatric Hematologist and Oncologist, who looks after children with immunodeficiency at KMC Hospital, said “Children who develop recurrent or multifocal tubercular or atypical mycobacterial infection the primary reason can be immunodeficiency and treatment must be planned accordingly.

Errors in genes weaken child's immunity. Inherited problems with T lymphocytes cause reduced ability to produce interferon gamma in response to mycobacteria and render the patients vulnerable to tubercular infections including live bacilli calmetter-Guerin (BCG) vaccines used to inoculate against tuberculosis.

In these children BCG vaccine can cause severe even fatal illness. He further added, “these above two cases highlight the genetic vulnerability to tuberculosis infection, this means affected children require prolonged treatment, some require preventive treatment, while a few benefit from addition of interferon gamma injections.”

Tuberculosis (TB) is a bacterial infection spread through inhaling tiny droplets from the coughs or sneezes of an infected person. It mainly affects the lungs, but it can affect any part of the body, including the tummy (abdomen), glands, bones and nervous system.

All though it is a serious condition, but it can be cured if treated with the right antibiotics. Currently around a million children suffer from TB worldwide and that more than 136,000 die each year. Inherited defect in the immunity leading tuberculosis infection is not well known.

Mangaluru: After as the state health ministry issued guidelines to the public in view of a heatwave, the health department in Dakshina Kannada has advised people to be cautious about the impact of heatstroke, including heat rash, heat edema, heat cramps, heat tetany, heat syncope, and heat exhaustion.

Dakshina Kannada district nodal officer for climate change, Dr Naveenchandra Kulal, said that the impact of heat-related illness can also be found in the district, when there is a variation of about 4.5⁰C from the normal temperature.

Dr Kulal told media persons here on Friday, that the impact of heat-related illness is not confined to Raichur, Kalaburagi, and other North Karnataka districts.

“Those who reside inside tin sheet houses and closed environments are likely to suffer from heatstroke. People should hydrate themselves by drinking more water and avoid working under the direct Sun. Heat exhaust fans can be used. Though no heatstroke cases have been reported in the district, any negligence may cause heatstroke,” he said. 

DHO Dr Thimmaiah HR said that hotels and restaurants should supply boiled water, and they should clean the overhead tanks once every three months. The filter of drinking water units should be cleaned frequently, he said.

He also directed paying guest accommodations to supply fresh food to inmates.

“We have received complaints of many paying guest accommodations serving sambar cooked the previous day to inmates. Stale food should be avoided, as there are chances of food contamination. Plastics should be avoided while packing food in hotels. As per the Food Safety and Standards Authority of India (FSSAI) guidelines, the use of plastic to pack hot food is restricted,” the DHO said.

Bengaluru, Apr 12: The two men who allegedly plotted and executed the blast that rocked Rameshwaram Cafe in Bengaluru last month have been arrested from Kanthi in Bengal's East Midnapore district, the National Investigation Agency said Friday morning. 

Mussavir Hussain Shazeb and Abdul Matheen Taha were caught after a joint operation by central intelligence agencies and police from Bengal, Karnataka, Telangana, and Kerala, and are en route to Kolkata, the anti-terror agency said in a statement.

The available evidence indicates Shazeb planted the explosive device, placed inside a backpack, at the popular eatery. Taha was responsible for planning the attack and for their disappearance.

These are the second and third arrests in this case; last month Muzammil Shareef, who extended logistical support to the Shazeb and Taha, was taken into custody.

Residents of Karnataka's Shivamogga district, Shazeb and Taha were traced to Kanthi after officials conducted searches at 18 locations across Karnataka, Tamil Nadu, and even Uttar Pradesh.

The blast at Bengaluru's Rameshwaram Cafe on March 1 injured 10 people, customers and staff.

Fortunately there were no deaths; the bag containing the explosives was placed in a relatively less crowded area and against a large pillar that absorbed the brunt of the explosion.

After the blast, the NIA released photos and videos of the accused, as seen on CCTV cameras across Bengaluru. In one such clip, the accused - wearing a face mask - was seen boarding a bus.

The agency had declared a reward of ₹ 10 lakh for information leading to the arrest of each accused. The agency had also questioned their acquaintances, including college and school friends.

The Rameshwaram Cafe, which suffered extensive damage after the blast, reopened eight days later, with enhanced security measures including metal detectors.

BJP’s reaction

The Karnataka BJP on Friday attacked the Siddaramaiah government after the arrest of two men by the NIA in the Bengaluru cafe blast case, saying that the terrorists were 'brothers' of the Congress.

Taking to social media, Karnataka BJP stated, “National Investigation Agency (NIA) detains ‘Brothers’ of Congress in Rameshwaram Blast case. Ever since Jihadi Tipu Sultan’s admirers came to power in Karnataka, terrorists have got a free ride.”

“Chief Minister Siddaramaiah’s ‘Ease of Doing Terror’ Policies have prompted ISIS to now set up its shop in Karnataka. The only guarantee that CM Siddaramaiah has fulfilled is converting prosperous Karnataka into a terror hub,” the BJP stated.

Mangaluru, Apr 13: Prime Minister Narendra Modi is all set to campaign for the upcoming and much awaited Lok Sabha election in coastal Karnataka by holding a mega roadshow in Mangaluru. 

Meanwhile, the organisers have announced a slight delay in the commencement of Modi's scheduled roadshow on April 14 in Mangaluru.

Addressing the media BJP principal secretary and Karnataka MLA V Sunil Kumar said: Originally the roadshow was scheduled to begin at 5 pm., but it will start at 7:30 pm."

As planned, the Roadshow will kick off from Narayan Guru Circle, where Prime Minister Modi will pay floral tribute to Sri Narayan Guru idol. From there, the Roadshow will proceed and conclude at Navabharat Circle. 

Mr Kumar urged people traveling to Mangaluru to see Mr Modi's visit by 7 p.m. and stand behind the barricades. The roadshow is estimated to conclude at 8:30 pm.
